Yes—sometimes. If someone is lactose intolerant, it does not automatically mean that every cake, brioche or pastry containing ordinary milk will cause symptoms.

But there is an important point I never blur: baking does not make ordinary milk lactose-free. The useful question is not only whether the recipe contains milk, but how much lactose the complete recipe contributes to one realistic portion.

What lactose intolerance actually means

Lactose is the natural sugar in milk. The small intestine normally uses the enzyme lactase to split lactose into glucose and galactose so they can be absorbed.

When lactase activity is low, some lactose can reach the colon without being fully digested. That can lead to bloating, gas, abdominal discomfort, diarrhoea or nausea. Tolerance is individual and dose matters; lactose intolerance is not automatically an all-or-nothing response to every trace of dairy.

Lactose intolerance is not a milk allergy

This distinction is essential in a bakery. Lactose intolerance concerns digestion of lactose, a carbohydrate. Milk allergy is an immune reaction to milk proteins.

Lactose-free cow’s milk still contains milk proteins. It may be useful for lactose intolerance, but it is not made safe for a person with a milk allergy simply because the lactose has been hydrolysed. When someone says milk allergy, I treat that as an allergen question, not a lactose calculation.

Does baking destroy lactose?

No. I do not use the oven as a lactose-removal method. Heating changes many parts of a batter or dough, and lactose can participate in browning reactions, but ordinary baking should not be described as making milk lactose-free.

So I would never tell someone that a pastry is suitable simply because the milk was baked. If ordinary milk went into the formula, I count it as a lactose source.

Why a person may still tolerate cake made with ordinary milk

The amount of milk in the whole recipe can be very different from the amount in one portion. Suppose a cake contains 250 g whole milk and is cut into 12 portions. If that milk contains roughly 5% lactose, it contributes about 12.5 g lactose to the complete cake, or roughly 1 g per portion from that milk.

That is an estimate, not a medical threshold and not a lactose-free claim. The exact milk specification and every other dairy ingredient still matter. But it explains why portion size is more informative than simply seeing milk on an ingredient list.

I calculate the complete formula, not just the milk

Milk may be only one lactose source. A pastry can also contain cream, milk chocolate, white chocolate, mascarpone, cream cheese, condensed milk, milk powder, whey powder or other dairy ingredients.

If I need an estimate, I calculate each lactose-containing ingredient from its actual specification, add the contributions and divide by the realistic finished yield. I do not calculate the liquid milk and then declare the complete dessert low in lactose.

Why milk powder deserves special attention

Liquid milk contains a large amount of water. Milk powder does not. Once the water is removed, the dairy solids—including lactose—are much more concentrated by weight.

That means 20 g milk powder is not equivalent to 20 g liquid milk from the point of view of lactose exposure. Whey powders and other concentrated dairy solids also need to be read from their actual specification rather than treated as a small, insignificant ingredient.

Butter is different from milk

Butter is primarily milk fat and normally contains far less lactose than liquid milk because much of the aqueous milk phase has been removed during manufacture. I therefore do not treat 100 g butter as equivalent to 100 g milk in a lactose calculation.

But butter is still a milk-derived ingredient, and product composition varies. If the lactose amount is important for a particular formulation or claim, I use the manufacturer’s specification rather than assuming zero.

What about cream and cheese?

Cream still contains lactose because it comes from milk, although its composition differs from ordinary milk because much more of its weight is fat. I do not assume that cream is automatically suitable for someone who is very sensitive to lactose.

Mature cheeses can contain very little lactose because lactose leaves with whey and is also metabolised during production and maturation. This is a useful reminder that 'contains dairy' and 'contains a lot of lactose' are not the same statement.

How much lactose can someone tolerate?

There is no single number I can promise for everyone. NIDDK notes that many people with lactose intolerance can tolerate some lactose, and many can tolerate about 12 g—the amount in roughly one cup of milk—without symptoms or with only mild symptoms. Individual tolerance can be lower or higher.

For pastry, the practical lesson is that one portion may contain much less lactose than a glass of milk. It is not permission for me to decide another person’s tolerance. When the person knows their own limit, a realistic per-portion estimate can help them make the decision.

The simplest substitution: lactose-free cow’s milk

When I want to preserve the function of dairy milk while greatly reducing its lactose, commercially prepared lactose-free cow’s milk is usually my first choice.

It still supplies water, milk proteins, minerals and, depending on the product, milk fat. Lactase has split the lactose into glucose and galactose. This preserves much more of the original milk system than replacing cow’s milk with an unrelated plant drink.

Does lactose-free milk behave differently in baking?

Slightly. Once lactose is split into glucose and galactose, the milk tastes sweeter even though extra table sugar has not necessarily been added. Those monosaccharides are also reducing sugars, so browning can change.

In many cakes, muffins and enriched doughs the difference is manageable. In a delicate pale preparation I pay more attention to colour. Technically, lactose-free milk is an excellent tool, but I still test the finished recipe rather than calling it completely identical to ordinary milk.

Can I replace milk with oat, soy or almond drink?

Sometimes, but that is a different formulation question. Plant drinks are not simply milk without lactose. Their protein, fat, carbohydrate, minerals, stabilisers and water-binding behaviour vary widely.

For a simple cake, a substitution may be easy. For brioche, pastry cream, custard, ice cream or an emulsion, milk proteins and solids can matter much more. I choose the replacement according to the job milk performs and test the complete preparation.

Lactose-free does not mean dairy-free

A pastry made with lactose-free cow’s milk still contains dairy and milk proteins. It is not vegan, and it is not suitable for a milk-protein allergy simply because the lactose has been reduced or hydrolysed.

This is one of the most important pieces of language to keep precise with customers: lactose-free, dairy-free and milk-allergy-safe are not interchangeable claims.

What about chocolate?

I check the exact chocolate. Dark chocolate may have no milk ingredient, but the words dark chocolate alone are not enough to prove that. Milk chocolate and conventional white chocolate contain dairy ingredients and can contribute lactose.

For professional work I read the ingredient and allergen information for the actual couverture and use the manufacturer’s specification when a lactose calculation matters.

What about fermented dairy?

Some fermented dairy foods may be better tolerated by some people with lactose intolerance, and their lactose content can differ from milk. That does not make every fermented dairy product lactose-free.

If I need a reliably reduced-lactose formulation, I prefer products specifically manufactured and labelled for that purpose rather than inferring suitability from fermentation alone.

Can I add lactase myself?

Lactase is the enzyme used to hydrolyse lactose into glucose and galactose, so in principle it can be used before a pastry is made. But enzyme treatment depends on dose, time, temperature and the dairy system.

For normal bakery production I prefer commercially prepared lactose-free dairy with a controlled specification rather than adding an arbitrary amount of lactase directly to cake batter and assuming the oven will finish the job.

How I estimate lactose per portion

For each relevant dairy ingredient I use: ingredient weight × lactose fraction = lactose contributed. Then I add all contributions and divide by the actual number of portions.

For example, 250 g milk at approximately 5% lactose contributes about 12.5 g to the complete formula. If the cake gives 12 portions, that is about 1.0 g per portion from the milk before I add any lactose from cream, chocolate, milk powder or other dairy ingredients.

This is a formulation estimate. It is not a substitute for product analysis, medical advice or the evidence required for a formal lactose-free claim.

My practical approach

If I am baking for someone who says they are lactose intolerant, I first clarify whether they mean lactose intolerance or a milk allergy. That single distinction changes the entire decision.

For lactose intolerance, I look at the complete formula and portion size. If I want to minimise uncertainty, I use lactose-free versions of the significant dairy ingredients. If I replace dairy entirely with plant-based ingredients, I treat that as a formulation change and check protein, fat, water, sweetness, browning and texture again.

My rule is simple: I calculate the ingredient in the portion, not just the ingredient in the recipe.
